What to Expect at Your First Dental Visit
Every situation and patient is unique, and we like to create priorities that are based on the patients’ needs and desires. We’ll make sure your first appointment leaves you feeling healthy and empowered to reach your oral health care goals.
Your initial examination is important for you to be able to communicate concerns you have, as well as gathering information for your care plan. We’ll work with you to fully understand your current state of oral health, and then, your dentist will discuss potential treatment options available to you. You’ll be informed and educated on the different directions your custom treatment plan may take. You will have ample time to ask questions and get the answers you need to feel confident in your care. Our caring team will help you feel at ease and relaxed through high-tech compassionate care. Before you leave, we’ll develop a plan for your ongoing oral health that will help you maintain your healthiest smile.
New Patient Forms
We are always excited to see new smiles coming through our door! If you’re brand new to Gary S. Barker, DMD, we’d like to take the opportunity to welcome you to our Pasadena, MD dental office. Our caring dentists and friendly dental team can’t wait to get to know you and your family.
Prior to your first appointment, we encourage you to complete our new patient forms electronically. Most patients find it much more convenient to provide the information in advance rather than in our dental office where you may not have easy access to necessary documentation.
If you have already provided a cell phone number or email address, you will receive a clickable link to fill out the new patient forms directly from your cell phone or email. If you prefer to print out the forms and fill them out by hand, please do so by clicking on each form below. If you would rather complete your new patient forms in our office, please plan to arrive 15 minutes prior to your appointment time.
Financial Options
Here at Dr. Gary Barker’s office, we want to help you achieve your goals and get the treatment you need. We are dedicated to making comprehensive, high quality dental care available to you and your family. In order to provide the highest quality of care, fees and financial arrangements will be discussed before any treatment is provided.
Any co-pays and fees will be due at the time of service, and we accept payments made with cash, checks and major credit cards. Our office is also in partnership with CareCredit to offer you a more flexible and affordable payment option.
Dental Insurance
We accept most PPO dental insurance plans. Our goal is to help you maximize your insurance benefits and make your remaining balance as affordable as possible.
Although patients are responsible for their insurance, as a courtesy, we will help you with your claim submission and reimbursement. As an extra courtesy for our patients, we also file insurance claims and evaluate denials.

Post-Op Forms

Brush Charts
Print off our brush charts provided by our dentist to help your kids with their oral hygiene at home! Our colorful and interactive charts make it fun for kids to brush their teeth, and it helps parents keep their children accountable for brushing.
